    Background
    CD74, also known as the invariant chain (Ii) of the major histocompatibility complex class II (MHC II), is a type II transmembrane glycoprotein that plays a central role in antigen presentation by serving as a chaperone for MHC II molecules. Its expression is primarily found in antigen-presenting cells such as B cells, monocytes/macrophages, and dendritic cells, though it is also present in T cells. Traditionally, CD74 is recognized for its function in facilitating the assembly and intracellular trafficking of MHC II complexes. Beyond its canonical role in antigen presentation, CD74 also participates in a range of MHC II–independent processes. It regulates endosomal trafficking and cell migration, and at the cell surface, it functions as a high-affinity receptor for macrophage migration inhibitory factor (MIF), a pro-inflammatory cytokine that acts as an upstream regulator in cardiovascular, infectious, autoimmune, and malignant diseases. CD74’s interaction with MIF initiates signaling cascades that modulate inflammation and cellular responses. In cancer biology, CD74 is frequently upregulated and has been linked to increased tumor proliferation and metastatic behavior. It is also expressed in several hematological malignancies, including multiple myeloma. Structurally, CD74 is a single-pass transmembrane protein with type II topology, placing its N-terminus in the cytoplasm. Through its diverse roles, CD74 influences signal transduction, membrane trafficking, and migration in antigen-presenting cells, while also contributing to disease pathogenesis and tumor progression.
