    Background
    UGT1A1 (UDP-glucuronosyltransferase 1A1) is a key enzyme of the UGT1 gene locus on chromosome 2q37 that detoxifies bilirubin and other lipophilic endogenous and exogenous compounds by conjugating them with glucuronic acid, enhancing solubility for excretion. Structurally, UGT1A1 shares conserved C-terminal sequences that mediate catalytic activity, while its variable N-terminal domain confers substrate specificity through alternative splicing of unique first exons. UGT1A1 is highly expressed in the liver and gastrointestinal tract, where it plays a crucial role in bilirubin clearance, drug metabolism, and protection against xenobiotic toxicity. Impaired or reduced UGT1A1 activity, due to genetic polymorphisms or regulatory defects, underlies conditions such as Gilbert’s syndrome, Crigler-Najjar syndrome, and drug-induced toxicities, highlighting its essential role in metabolism and clinical pharmacology.
