Tersolisib (STX-478) is a potent, mutant-selective, allosteric inhibitor of phosphoinositide 3-kinase alpha (PI3Kα) that targets prevalent helical- and kinase-domain mutant variants, including HR [1, 2]. By selectively inhibiting mutant PI3Kα over wild-type PI3Kα, it suppresses hyperactive AKT/mTOR signaling to impair tumor growth in mutant xenografts while sparing metabolic dysfunction [1, 2]. Furthermore, combining Tersolisib with fulvestrant or CDK4/6 inhibitors promotes durable tumor regression in ER-positive breast cancer models [1, 2]. [1]
