Mechanism and Biochemical Profile

Pretomanid (PA-824) is activated by mycobacterial deazaflavin-dependent nitroreductase to generate reactive nitrogen species, which inhibit mycolic acid biosynthesis and disrupt bacterial respiratory chain function. This dual metabolic inhibition causes severe cell wall degradation and intracellular ATP depletion, driving potent bactericidal and sterilizing activity against *Mycobacterium tuberculosis* in clinical regimens for drug-susceptible and drug-resistant pulmonary tuberculosis.
