Triamterene is an epithelial sodium channel (ENaC) blocker that inhibits ENaC in a voltage-dependent manner with an IC50 of. By directly suppressing sodium ion influx through apical ENaC complexes in renal epithelial cells, triamterene reduces lumen-negative potential and transepithelial sodium reabsorption. Downstream, this impaired sodium transport decreases electrochemical driving forces for potassium secretion, promoting natriuresis while preserving systemic potassium levels. [1]
