Etofibrate is a hybrid hypolipidemic compound combining clofibrate and niacin moieties that acts as a peroxisome proliferator-activated receptor alpha (PPARα) agonist. Upon cleavage into clofibric acid and nicotinic acid, it modulates PPARα-mediated transcription to regulate lipid and lipoprotein metabolism. This activation promotes fatty acid oxidation, reduces cellular triglyceride synthesis, and regulates lipid transport pathways through nuclear receptor target gene expression. [1]
