Clarithromycin is a macrolide antibiotic and CYP3A4 inhibitor that targets bacterial protein synthesis. It reversibly binds to the 50S ribosomal subunit of susceptible microorganisms, inhibiting translocation of aminoacyl-tRNA and blocking peptide elongation. This action halts bacterial protein translation, conferring antimicrobial activity against pathogens including Legionella pneumophila. Additionally, clarithromycin acts as a substrate and inhibitor of cytochrome P450 isoform CYP3A4. [1]
