Mechanism and Biochemical Profile

Aprepitant selectively binds to the neurokinin-1 (NK1) receptor with high affinity, thereby blocking substance P-mediated signal transduction and reducing downstream expression of pro-inflammatory cytokines such as IL-6, IL-8, and TNF-alpha. By suppressing NK1 receptor activation within both the central nervous system emetic center and the gastrointestinal tract, the drug attenuates vomiting reflexes, providing effective clinical prophylaxis against chemotherapy-induced nausea and vomiting.
