Clinical Trials

Merck Sharp & Dohme LLC has sponsored several clinical trials evaluating verubecestat (MK-8931) across Phase 1, Phase 2, and Phase 3 for neurodegenerative conditions, including Alzheimer's disease, amnestic mild cognitive impairment, and prodromal Alzheimer's disease. Early-phase studies successfully completed safety, pharmacokinetic, and pharmacodynamic evaluations in patient populations with renal or hepatic impairment as well as Alzheimer's disease. However, major late-stage Phase 2/3 and Phase 3 efficacy trials focusing on mild-to-moderate and prodromal Alzheimer's disease were ultimately terminated.

Compliance for Clinical Use

Mechanism and Biochemical Profile

Verubecestat (MK-8931) potently and selectively binds to beta-site APP-cleaving enzyme 1 (BACE1), thereby blocking the rate-limiting endoproteolytic cleavage of amyloid precursor protein and suppressing downstream generation of neurotoxic amyloid-beta peptides. By halting amyloid-beta production and neurotoxic plaque accumulation, this mechanism directly addresses the primary pathological driver targeted in clinical trials for Alzheimer's disease, prodromal Alzheimer's disease, and amnestic mild cognitive impairment.

Appendix RUO and cGMP Quality Standards

Quality Dimension RUO (Research Use Only) cGMP (Current Good Manufacturing Practice)
Clinical Applicability Prohibited in human clinical trials or medical diagnostics. Mandatory for human clinical trials (Phase I–III) and therapies.
Regulatory Status Non-regulated grade; exempt from drug manufacturing laws. Legally enforced by health authorities (e.g., FDA, EMA, NMPA).
Facility Environment Unclassified analytical or research laboratories. Validated Cleanrooms (ISO Class 5–8) with continuous monitoring.
Quality Control Basic purity and activity testing. Rigorous safety release testing (Sterility, Endotoxin, Mycoplasma).
Process Validation Basic equipment calibration; no process validation required. Full qualification (IQ/OQ/PQ) and complete batch records.
Quality Assurance Vendor self-declared without required formal QMS. Mandatory QA/QC unit, Change Control, CAPA, and vendor audits.
Regulatory Impact High risk of IND rejection if used as a critical raw material. Required for IND/NDA filings, supported by Drug Master Files (DMF).
