Tienilic acid is a uricosuric diuretic agent and a mechanism-based inactivator of cytochrome P450 2C9 (CYP2C9). The compound undergoes CYP2C9-mediated oxidative bioactivation to produce reactive electrophilic thiophene sulfoxide intermediates. These reactive metabolites covalently modify target hepatic microsomal proteins, resulting in covalent protein adduct formation and subsequent downstream immune-mediated cellular toxicity and hepatotoxicity.
