Roxadustat (FG-4592) is an orally active hypoxia-inducible factor (HIF) prolyl hydroxylase inhibitor. By inhibiting HIF prolyl hydroxylase enzymes, Roxadustat stabilizes HIF-1α and HIF-2α transcription factor subunits, preventing their degradation and promoting functional HIF complex activation. This response directly upregulates the transcription of genes essential for erythropoiesis and iron metabolism, including erythropoietin (EPO), thereby promoting red blood cell production and modulating cellular oxygen-sensing pathways.
