Aceclidine is a quinuclidine-based cholinergic agonist targeting muscarinic acetylcholine receptors M1–M5, functioning as an M1 receptor agonist and M3 receptor modulator. Activation of M1, M3, and M5 receptors stimulates phosphoinositide hydrolysis, whereas M2 and M4 activation inhibits cyclic AMP accumulation. In ocular tissue, Aceclidine acts as a pupil-selective miotic that contracts the iris sphincter muscle to produce a pinhole pupil, extending depth of focus with minimal ciliary muscle stimulation.
