    Background
    Mucin 5AC (MUC5AC) is a secreted, gel-forming mucin glycoprotein belonging to the mucin family, predominantly expressed by goblet cells in the respiratory, gastric, and conjunctival epithelia, where it constitutes the primary component of protective mucus gels. MUC5AC is a massive multidomain protein featuring N- and C-terminal globular regions with von Willebrand factor (VWF)-like D-domains (D1, D2, D2', D3, D4), cysteine-rich subdomains (CysD1-9), and a central proline/threonine/serine-rich (PTS) mucin domain densely O-glycosylated (>80% mass as carbohydrate) that extends into rigid rod-like structures; N-terminal D1-D3 domains mediate multimerization into tightly branched networks via disulfide bonds and homotypic interactions. MUC5AC polymerizes with MUC5B to form viscoelastic mucus layers that trap pathogens, facilitate mucociliary clearance, lubricate epithelia, and bind water for barrier hydration; its biophysical properties create stiffer, denser gels compared to MUC5B due to extensive branching. MUC5AC expression is upregulated via EGFR and IL-13/STAT6 pathways during inflammation, contributing to airway obstruction in asthma, COPD, and cystic fibrosis, where hypersecretion impairs clearance; it also promotes tumor progression in lung adenocarcinoma via ANXA2 signaling.
