PET imaging for chemokine receptor-4 detection in multiple myeloma

 

Chemokine receptor-4 (CXCR4) is one of the member of the G-protein coupled chemokine receptors. It activated by SDF-1, its natural ligand, and trigger downstream signaling pathways, such as MAPK and PI3K signaling pathway, to regulate blood cell adhesion, migration and homing. Pathological CXCR4 overexpression has been reported as an adverse prognostic factor in various types of solid cancers, as well as hematopoietic malignancies. Philipp-Abbrederis et al. developed a novel CXCR4 probe [68Ga]Pentixafor for mapping of CXCR4 expression density in vivo. The article was published in EMBO Molecular Medicine.

 

The study was conducted in mice xenografted with human CXCR4-positive multiple myeloma (MM) cell lines and patients with advanced MM by an approach called positron emission tomography (PET). Among 25 patients, 14 of them were identified with CXCR4-positive MM. 10 of 14 CXCR4-positive MM patients were revealed MM positive by using [68Ga]Pentixafor PET/CT scans, whereas 9 of 14 patients were rated MM positive by using standard [18F]fluorodeoxyglucose PET/CT scans. According to the results of blood counts and standard CD34+ flow cytometry, blood count did not significantly change after tracer application. The findings indicate that CXCR4-directed [68Ga]Pentixafor PET tracer broaden the field for clinical investigation and therapeutic examination of CXCR4-positive MM and other malignancies.
